Please fill out the yellow highlighted sections and bring completed form with you once you to one of our scheduled clinics or appointment. If you have any insurance coverage, please have your current active insurance card(s) with you at the time of vaccination to verify eligibility. Please note: You will not be charged or receive a bill for the allowed billable vaccination administration charge regardless if you have insurance or not. The vaccine itself is free of charge.
